Proctor Nomination

Important Information for Proctor Candidates

You have been nominated to serve as a proctor for an examination that he or she will be taking shortly. This form explains the proctoring process and serves to verify that you agree to assist the student with this requirement.

If you are approved as the student’s proctor, you will receive an email containing online exam access instructions and the student’s online exam log in password. The exam is a timed (3-hour), closed-book examination. Students cannot take the exam while accessing books, notes, internet sites, or reference aids of any kind.

You will need to contact the student to confirm the date, time, and location of the exam. Before you allow the student to begin the exam, you will need to make sure that three hours are available and confirm the student’s identity with a government-issued photo identification.

Before the exam begins, you should advise the student that no breaks are permitted during the exam.

Please note that you cannot serve as a proctor for this exam if any conflict of interest exists. For example, if you are related to the student, an immediate supervisor, a fiancé, or roommate.

Please complete the following information. Completion of this form indicates that you are willing to proctor the student’s exam for ACHS and adhere to ACHS’s proctoring requirements. If you have questions about the process or your suitability to serve as a proctor, please call (800) 487-8839 and speak to Student Services or email proctor@achs.edu. We thank you in advance for providing this important service.
