Please Request Your Registration Summary

**Please submit the form below to request a new copy of your upcoming ACHS Semester Registration Summary for review and to finalize your registration. **

**By submitting this form, you will receive a copy of your current registration summary for the upcoming semester.** Registration for upcoming semesters generally opens on the following schedule: October 1 for January/March semesters, February 1 for May/June semesters, and June 1 for September/October semesters.
