ACHS wants to celebrate your success by publishing the results of your thesis or Capstone project. This is a great way to promote your work in the world and serves as a springboard for future publication and professional opportunities!

Please complete the following form to grant ACHS permission to publish your thesis or results of your Capstone project with appropriate credit. Your thesis will be published with Open

Access meaning that your thesis will be immediately made public via the ACHS website and may be shared via social media and other public outreach venues.

The following consent form was prepared by the American College of Healthcare Sciences (ACHS) for the purpose of signee consent to publish your student thesis project on the Internet or in print as applicable. By completing this electronic form:

I understand that publishing my thesis or Capstone project is voluntary, will not be compensated, and will be appropriately credited to me.
I understand that my student thesis or Capstone project will be published and may be promoted via ACHS social media venues and other media outlets at ACHS's discretions (with appropriate credit cited).

By signing below, I am affirming that I am the sole owner/author of the content of my submitted thesis document or Capstone project and that the work, in whole or in part, is not subject to copyright by any other parties, and I am granting permission to American College of Healthcare Sciences to publish my thesis or Capstone project online or in print.
