This online assessment is completed by current students interested in transferring to a new course or program. Please note that students in all programs must the ACHS minimum eligibility requirements to be considered for a new program.

Your responses will be considered by the Admissions Committee who will make the final decision on the acceptance of your transfer application. We encourage you to include detailed responses that will provide the Admissions Committee with adequate information to evaluate your application for transfer. Please limit responses to 200 words or less for short essay questions.
