# ACHS Incomplete Grade Request Form

We know that challenges may arise that impact your studies. If you will not be able to complete your course by the semester end date, we want to support you to be successful. 

Please complete the following form and submit your $49 fee to let us know that you intend to complete the remaining coursework in your incomplete course(s), and we'll work together on a plan that will help you be successful!
